42nd Dance-A-Rama Registration

Please use a separate form for each dancer!  Complete Weekend Only!
Registration Fee includes Dance Program Events for Aug 31-Sept. 2 and provides complimentary membership to SSDUSA from 9/3/12 – 9/1/13.  Souvenirs, Tours & Trail In Dances are extra.
Advanced Registration (before Aug. 1, 2012): $40 per person       

Cancellation Fee: $5.00 after June 1, 2012

Regular Registration (after Aug. 1, 2012): $45 per person        

NO Refunds after Aug 1, 2012

Meal tickets are available for purchase your breakfast and dinner in advance. The deadline to purchase meal tickets is August 15, 2012. A mobile lunch cart or the Hotel restaurant will be available for lunch.
